Description
This process will aid your Human Resources department in tracking employee FMLA (Family and Medical Leave Act) leaves of absence. Assigned tasks, automated reminders, and date calculations allow for easy tracking, and ensure that all necessary information is captured and stored.
Participants
Human Resources Staff: Sends emails, updates date fields, and manages the FMLA leave process
Employee: Submits the form, fills out the relevant personal information, and uploads/provides necessary documentation (e.g. medical certificates, birth certificates) needed to designate FMLA leave
Laserfiche Repository
Submitted forms and uploaded documents will be saved in the Business Process Library\FMLA Leave Tracking\ folder, in one of three sub-folders (depending on the result of the process): Processed Requests, Rejected Requests, or Terminated Requests. Forms will be saved in a separate employee sub-folder, following the naming convention [Last Name], [First Name].

This process is intended for FMLA Tracking only.: This process is meant to aid in the tracking of only FMLA leaves, and not generic vacation or sick leaves. Please add to the process to add non-FMLA leaves, or use a separate process.
