Student Health Information Form

This process starts when a parent fills out the form and provides information about the student's doctor and any special dietary accommodations necessary for the student. The questions on the form comprise of "Yes" or "No" answers. If "Yes" is chosen a box opens up for explanation for that question. The form is then sent for approval by the school secretary. Once approved, if the dietary accommodations button is checked, an email is sent to the food services department with the student information.
Also included is a Spanish version of the form. Once submitted, it sends the same form in English to the secretary for approval. From then on, the form is in English so it does not have to be translated any further. Please see annotation in process diagram on how to configure the process to include the Spanish form.
