Published in line with the American Rescue Plan "ARP" Guideline. This process is designed to be a simple and well tested mechanism to perform contact tracing across an entire organization. A screening questionnaire is filled out by the submitter and can be sent to a Health Services team for review. This form follows current American Medical Association (AMA) and Center for Disease Control and Prevention (CDC) COVID-19 guidelines, but has been built to easily support adding/removing additional screening questions as well to be general enough for any viral contagion.
