This solution is intuitive and easy for employees to submit the request digitally and upload their supporting documents securely. Business process tools allow health services and human resources departments to review an exemption request. This process can be used in tandem with the COVID-19 Test Result Submission Process to enroll the requester into a weekly testing program. Collected request forms and supporting documents are stored under records management security and retention policies to align with OSHA requirements.

Participants
Human Resources: Reviews submitted request forms to approve, reject, or request clarifications and more supporting documents from employees
Employee: Fills out the initial vaccine exemption request form with supporting documents
