A digital form can be automatically emailed to employees at the end of each week by Laserfiche Workflow. Employees can submit their weekly test results with their personal information via the digital form.

Employee: Uploads test results weekly
Human Resources: Monitors the submission status of unvaccinated employees in the Laserfiche Forms portal

All submitted forms and uploads will be saved to the repository using the following path: \Solution Templates\COVID-19 Testing\[Last Name], [First Name] - #[Employee ID]\Test Results
