The Student COVID-19 Vaccination Records Request Process enables institutions of higher education to systematically collect student vaccination status required to facilitate safe campus reopening. This process is initiated when a student fills out an intuitive digital form to self-report vaccination status and securely upload their government-issued proof of vaccination. This solution can be used with additional workflows based on vaccination status, such as enrolling unvaccinated students in a weekly testing program, scheduling vaccination appointment for second dose and obtaining their vaccine exemption requests for religious or medical grounds. The submission form and uploaded documents are automatically saved to the Laserfiche repository under records management security and retention policies to align with OSHA requirements.
