This process helps office administrative staff request access for new employees for email, a phone, the enterprise ERP, and other various systems. This form is triggered after a new employee personnel action form is reviewed and approved by Human Resources. Many of the fields on the form come pre-populated from the previous process. Once submitted, an IT Help desk request is created to set up an Active Directory Account. Once the Active Directory account is set up, the relevant IT help desk tickets are created for phone and other various systems. A final email is sent to the requester to let them know these requests have been made and are notified once completed. This form also initiates 30 and 60 day new-hire check-in reminders with Human Resources. Completed forms are not saved as they are not needed for future reference.
