Enables K-12 schools and school districts to collect information about the vaccination status of students. This process is initiated when the student submits COVID-19 vaccination status and upload an image of their proof of vaccination through an electronic form (parents can help with the submission). If students are partially vaccinated, they can make a vaccination appointment. If there are extraneous reasons for not receiving the vaccination, they can also submit their form with their doctor’s certificate, or supplemental information for religious or other reasons. The form will be routed to a health specialist in one of the two ways. First, for fully or partially vaccinated students, the specialist confirms their vaccination proof and reviews their vaccination appointment schedule. Second, for non-vaccinated students, the specialist confirms their exemption reasons, enrolls them into weekly COVID-19 test and reviews their vaccination appointment schedule. As forms are submitted, they will be saved to the repository along with all supplementary documents.
