This form and business process allows for constituents and visitors to the city report instances of acts of discrimination. The form contains translated fields for all the questions in 5 languages (English, Spanish, French, Portuguese and Haitian Creole), and the final page of the form, which is hidden, captures the field values from each translated field and promotes the appropriate one as the "Final" for each for ease of reporting, or data consumption. Any error messages created during the form submission are also promoted to the user in the language that they invoked.
